About the Role
The D2A (Discharge to Assess) service provides short‑term, high‑quality support to individuals who are well enough not to need an acute hospital bed but do need some extra support in their home. You will assess customers’ needs, motivate them, support with tasks around the home and personal care, and work a mixture of early and late shifts between 7 am and 10 pm, alternating weekends.
* Assess customers’ needs to identify areas where support is needed to promote independent living.
* Motivate customers to build their confidence so they can achieve the goals they have set and return to independent living.
* Support customers with tasks around the home and with personal care such as personal hygiene, toileting, and dressing.
* Work a mixture of early and late shifts between 7 am and 10 pm and alternate weekends.
About You
You are passionate and experienced, with a background in care and hold your QCF Level 2 in Health and Social Care. You will have a full driving licence and access to your own vehicle, as you will need to travel between clients. Unfortunately, we are unable to accept applicants requiring sponsorship for this position.
