Health Care Support Worker (Adult Crisis and Home Treatment Team)
We are a well-established community mental health team looking to recruit a Health Care Support Worker to join our Crisis and Home Treatment Resolution team.

Role Overview
The role of the support worker is to complement the community mental health multi-disciplinary team in delivering interventions and support to promote the recovery of people experiencing mental health difficulties. This is a fixed-term/secondment for 12 months.
Responsibilities
* Support senior mental health practitioners to deliver responsive and effective care during assessment and treatment to a designated caseload of individuals experiencing moderate to severe mental health difficulties.
* Work in a compassionate and non-judgemental manner with individuals in their own homes, in the community and in health settings.
* Accurately record contacts with individuals in clinical notes in a timely manner.
* Support the senior practitioner to complete all necessary administrative tasks.
Qualifications
* Enthusiastic about supporting people experiencing moderate to severe mental health difficulties in a compassionate and holistic manner.
* Experience or training in supporting therapeutic interventions preferred.
* Good written and verbal communication skills in English. The ability to speak Welsh is desirable.

Services Provided In
Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest.
Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire.
Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings.
