We support adults whose health and wellbeing are affected by their housing situation or home environment, especially those living with frailty, multiple conditions, or complex needs. The Role As a Social Prescriber, you’ll be the bridge between individuals, families, and the services that can help them. You’ll work closely with clients to understand their needs, build confidence, and connect them with the right community, voluntary, and public sector support. Key Responsibilities · Receive referrals from health, social care, and community partners · Develop Personalised Care and Support Plans (PCSPs) tailored to client needs and goals · Provide advice and guidance using motivational interviewing and MECC techniques · Empower clients to make informed decisions and build independence · Manage a caseload effectively, keeping accurate records and data · Signpost and refer clients to voluntary, community, and statutory services · Identify gaps in local services and report barriers to outcomes · Build strong relationships with local organisations and partners · Work flexibly across homes, community settings, and offices What we’re looking for · Experience in advice delivery, coaching, or motivational interviewing · Strong communication and organisational skills · Ability to work independently and as part of a team · IT Skills (Word, databases, spreadsheets) · Empathy, adaptability, and attention to detail What We Offer · Comprehensive induction and ongoing professional development · 4.5% employer pension contribution · Flexible working opportunities · 25 days annual leave (including office closures: 3 days at Christmas and 2 in August) plus bank holidays A full training program will be given.
