Overview
The Outreach practitioner role is to work in an outreach service within the health and justice service. The service supports vulnerable children and adults who have entered the criminal justice system, with a focus on those who have been released from police custody and may have additional health needs. The team provides support for up to 12 weeks in the community to reduce reoffending risks and health vulnerabilities. The role offers opportunities to develop pathways of support for children and young people and to build connections with partner agencies.
Main responsibilities
* Work within the outreach service with a focus on adults and children/young people referred after contact with criminal justice services.
* Develop pathways of support in partnership with agencies and contribute to creating new pathways of support for children and young people.
* Support the outreach service to develop a triage and referral pathway for adults, children, and young people, with oversight alongside the Team Leader.

Job description and responsibilities
Main duties and responsibilities include:
* Take responsibility for specialist assessment (within service specification) in Liaison and Diversion, in conjunction with the service user and carer, promoting self-determination and autonomy.
* Prepare written reports for Crown or Magistrates Courts or attend and provide verbal evidence as required; offer recommendations to these bodies.
* Participate in setting quality standards, auditing, monitoring and reviewing practice in line with current clinical guidance and policy.
* Assess patients' individual care needs, agree and refer to appropriate agencies; complete and contribute to robust risk assessments and risk management plans.
* Deliver and promote positive risk-taking to improve quality of life and maximise independence.
* Maintain an effective reporting system, including verbal and written reports on patient conditions.
* Utilise agreed outcome measures to review effectiveness of processes as part of the Health and Justice Service.
* Support clinical governance processes at team level and assist in achieving the service aims and objectives set by the Team Leader.
* Adhere to the NMC Code of Practice and professional guidelines; maintain up-to-date professional competence; participate in research and other projects as required.

Qualifications and experience
Essential
* Registered Mental Health Nurse, Learning Disability Nurse, or General Nurse on the NMC Register or Registered Social Worker.
* Post-registration experience with formal courses and training at diploma/degree level.
* Extensive post-registration experience working with clients with mental health-related difficulties.
* Experience of working in community settings and making autonomous decisions; experience of coordinating care across multidisciplinary and multi-agency teams; ability to carry out clinical audits.
Desirable
* Leadership qualification; BSc degree in nursing or equivalent.
* Psychological intervention qualifications (e.g., EMDR, CBT, CBTp, Family Interventions, CAT, DBT, Structured Clinical Management).
* Lived experience of the criminal justice system, care system, or vulnerabilities.
Skills, knowledge and abilities
* Knowledge of Mental Health NICE guidelines and the NMC Code of Conduct.
* Understanding of the Care Programme Approach and Community Care Act; equality and anti-discriminatory practice.
* Knowledge of statutory, independent and third-sector services for adults with mental ill health.
* Skills in assessment, planning and evaluating care; risk assessment and risk management planning.
* Holistic assessment of mental health, vulnerabilities, strengths and needs; multi-disciplinary and agency interventions.
* Ability to interpret physical health investigations and provide health promotion advice (weight, BMI, blood pressure, smoking, alcohol, drugs).
* Knowledge of Mental Health Act, Mental Capacity Act and Deprivation of Liberty Safeguards; strong interpersonal skills and ability to work collaboratively with service users, carers and agencies.
