Job Title: Social Worker - Community Mental Health Team - Rochdale Location: Rochdale Pay Rate: £30 Liquid Personnel is currently recruiting for a Social Worker to join a Community Mental Health Team based in Rochdale. What will your responsibilities be? Undertake statutory adult social care functions within a secondary mental health service, working with individuals experiencing severe and enduring mental ill-health. Complete strengths-based assessments, scheduled and unscheduled reviews, care planning, and commissioning of support in line with individual needs. Carry out Mental Capacity Assessments, Section 42 safeguarding enquiries, and facilitate timely hospital discharges in collaboration with health colleagues. Apply sound knowledge of key legislation, including the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, and associated frameworks such as DoLS, CHC, and data protection regulations.
