We are currently looking for an enthusiastic and motivated Locum Social Worker to join a busy Hospital Discharge to Assess team in the Greater Manchester area.
The role will be either based within a fast paced Hospital Discharge Team. The team is made up of a team of dedicated health and social care staff, working together to provide holistic assessment and support plans to ensure the best possible outcomes.

Responsibilities:
8. Put plans in place to coordinate a safe and timely discharge or in a community setting who are needing an urgent assessment to reduce the risk of being admitted.
9. You will carry out assessments under the Care Act 2014 to determine eligibility need
10. Maximise rehabilitation services such as reablement & care where appropriate
11. Provide assessment and appropriate interventions to aid patients in achieving optimum recovery/rehabilitation and quality of life.
12. Work with patients and families addressing the impact of illness and treatment
To be considered for the Senior Social Worker role:
13. 1-3 years experience of working within a similar hospital, urgent care or community setting
14. Diploma/Degree in Social Work or equivalent
15. Social Work England registration
16. Experience of working under the care act and mental capacity act
