Pertemps Social Care is looking for a passionate Social Worker to join our clients Hospital Discharge Team. You’ll help adults safely transition from hospital to home, working closely with health teams and using your skills in assessment, planning, and risk management.
✅ Must be SWE registered
✅ Experience in adult social care & hospital discharge
✅ Car driver essential
