An exciting opportunity has arisen for an experienced long term condition clinical lead nurse to join a busy multidisciplinary team at HMP Deerbolt.
The post holder will practice independently and autonomously delivering care and treatment to patients at HMP Deerbolt. The post holder will have gained a wide range of knowledge and clinical skills in a Primary care setting. We aim to be flexible in matching your availability with our work patterns and the needs of the service and therefore we are also able to offer part time and flexible working options.
The role delivers a high standard of care on the treatment of minor illness and minor injury and the post holder will have expertise in the management of long-term conditions such as Diabetes, COPD, asthma, heart disease and support cancer screening services. The post holder will be experienced in the delivery of Public Health England’s health promotion programs such as Immunisations, women\'s health, and lifestyle conditions such as smoking and obesity.
The long term condition clinical lead nurse will have management, leadership and development responsibilities for the healthcare team.
The post holder will have excellent organisational and time management skills to ensure clinics and services are managed efficiently and effectively, without compromising patient care or safety.
Responsibilities
* Deliver a high standard of care on the treatment of minor illness and minor injury.
* Provide management of long-term conditions such as diabetes, COPD, asthma, and heart disease and support cancer screening services.
* Deliver Public Health England health promotion programs such as immunisations, women\'s health, and lifestyle interventions (smoking and obesity).
* Lead and develop the healthcare team with management, leadership and development responsibilities.
* Ensure clinics and services are managed efficiently and effectively with excellent organisational and time management skills, without compromising patient care or safety.
Requirements
Evidence of substantial clinical experience in primary care, with expertise in long-term condition management and health promotion.
