What the Job Involves The role involves managing an allocated caseload and performing comprehensive assessments and interventions under the all-Wales pledge. You'll work with individuals needing care and support to assess risks, develop care plans, and ensure appropriate support is provided. You will be involved in designing care, support, and education plans, and recommending external provisions within budget constraints. The role requires effective collaboration with various service providers to optimize resources and ensure necessary approvals. You will also participate in a duty system on a rota basis, responding to immediate crises effectively and proportionately while managing and recording potential risks.
