Work as a social worker within a multi-disciplinary team, delivering a service to individuals with severe and enduring mental health problems or dementia.
Responsibilities
* Work as a social worker within a multi-disciplinary team, delivering services to individuals with severe and enduring mental health problems or dementia.
* Operate as a statutory role under the Mental Health Act 1983, Mental Capacity Act 2005, Social Services and Well-being Act 2014 and the Mental Health (Wales) Measure 2010.
* Adopt the guiding principles of the Care and Treatment Planning framework.
* Conduct s.126 enquiries and safeguard the rights of those in care.
* Liaise effectively with other mental health service providers to facilitate seamless delivery of services.
* Promote the social inclusion of people experiencing serious mental illness.
* Advance a recovery model of care so individuals can lead independent and fulfilled lives.
Qualifications
* Provide evidence of all essential qualifications.
* Desirable: Ability to speak Welsh.
