We are looking to recruit a dedicated, enthusiastic, andmotivated Advanced Nurse Practitioner to join our friendly team based atHerrington Medical Centre, Houghton Le Spring.
The post holder will be caring, compassionate andexperienced Practitioner who, acting within their professional boundaries, willprovide care for our patients including initial history taking, clinicalassessment, diagnosis, treatment, and evaluation of care. They will demonstrate safe, competentclinical decision-making and expert care, including assessment and diagnosticskills, for patients within the practice.
The post holder will communicate and work collaborativelywith the Practice Manager and GPs, as well as our practice team, to meet theneeds of patients, supporting the delivery of policies and procedures.
Main duties of the job
To be responsible for the delivery of a high standard ofpatient care as Advanced Nurse Practitioner in General Practice, usingadvanced, autonomous clinical skills and a broad in-depth theoretical knowledgebase. To manage a clinical caseload, dealing with presenting patients' needs ina Primary Care setting. The Advanced Nurse Practitioner will need to prioritiseand triage the needs of patients accordingly making any necessary referrals tosecondary services for diagnosis and treatment.

We have approximately 9500 patients and are based atHerrington Medical Centre. The team consists of 5 GP Partners, a Salaried GP, 1Advanced Clinical Practitioner, 3 Practice Nurses, 2 HCAs, 1 SNA with a strongand supporting Administration Team. Weare a GP training practice, and host registrars, Medical Students and NursingStudents. The practice works collectively with The Primary Care Network andARRS role. The practice was CQC inspected in 2020 and was awarded Outstanding.

* Provide the choice of direct access to anAdvanced Nurse Practitioner, both in the practice, over the telephone and viahome visits for the general practice population.
* Make professionally autonomous decisions forwhich post holder is accountable.

* Provide a first point of contact within thePractice for patients presenting, both in the Practice and over the phone, forpatients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physicalexamination, problems solving and clinical decision making, to establisha diagnosis and care management plan.

* QUALIFICATIONS
* Registered General Nurse MSc or equivalent
* Advanced Clinical Practitioner qualification or equivalent experience

* Clinical Practitioner - management of minor illness and ailments
