The role of the Care Co-ordinator is pivotal within primary care as the interface between service users, carers, primary care, secondary care, community care, social care, mental health, out of hours and voluntary organisations. The Care Co-ordinator will be part of the Belper Integrated Community team and working alongside the Belper GP Practices. The Care Co-ordinators provide an administrative, non clinical role, and applications are welcome from anyone with Health or primary care experience. We are looking for excellent communicators with good time management skills who have the enthusiasm and motivation to improve people’s lives.
