Job Summary
Consultant Practitioner with clinical expertise in frailty? We are seeking an expert clinician to lead on a number of system priorities, including Frailty Hospital at Home, and clinical support with the development of Integrated Neighbourhood Teams. This is an exciting time to join as we further develop our Frailty Hospital at Home in partnership with our colleagues in University Hospitals Dorset, with a 'one team' approach. The post holder will work alongside acute clinicians and teams to deliver the flagship service across the Bournemouth, Christchurch and Poole area. The role is full‑time, based in Bournemouth and Poole, requiring flexible working across the conurbation.
Responsibilities
* Work collaboratively with the multi‑professional team, practice autonomously while being accountable and self‑directed in line with the relevant code of professional conduct.
* Provide clinical input and/or consultative sessions within a defined cohort of patients and service users, with a minimum of 50% of the portfolio in direct autonomous clinical practice.
* Lead as a driver for change, monitor and improve standards of care, safeguarding and clinical effectiveness through supervision, evidence‑based practice, clinical audit, research, and education.
* Maintain and develop a local and national profile as a Consultant Practitioner, being a creative and innovative leader in developing services for the relevant client group.
* Advise and communicate with acute hospitals, primary and social care, and community teams to ensure seamless continuity and transfer of care.
* Ensure Enhanced DBS check, with the Trust covering the cost.
Qualifications
* Degree or equivalent level registered health practitioner.
* Evidence of recent evidence‑based professional development demonstrating achievements in practice/speciality, leadership, education and service development.
* Registered non‑medical prescriber (if professionally appropriate).
* Clear and demonstrable evidence of advanced, autonomous practice in the management of patients/service users.
* Learning and Assessing in Practice qualification or equivalent practice assessor training.
* Evidence of proactive involvement in the supervision and development of members of the multi‑disciplinary team and ability to demonstrate changes of practice through this.
* Wide knowledge of health policy and its application in practice, evidence of influencing local and national standards.
* Advanced skills for assessing and interpreting complex client conditions, comparing options and taking appropriate action.
* Experience of coordinating complex activities/programmes of care with other professionals and agencies.
