Summary:
To provide 24 hour supervisory nursing care to ensure appropriate patient care.
Responsibilities:
Ensure patients are assessed and evaluated as needed for change in conditions to ensure appropriateness of re-hospitalizations. Ensure SBAR's and rapid response and any additional interventions are implemented as needed.Assist with clinical needs of patients and support nursing staff as needed including but not limited to:- Assisting with IVs- Assisting with blood draws- Orders are carried out- Call light- Treatments and Medications- Showers- Fall and Skin InterventionsEnsure nursing documentation is completed including but not limited to:- MAR- TAR- Nurses Notes- Wound- 24 Hour chart check- QAPI AuditsCompletes patient care requirements by scheduling and assigning nursing and staff; following up on work results and ensure customer service standards.Ensures Admission protocol is followed including but not limited to:- Admission paperwork is completed timely, accurately and fully completed- Complete new admit chart reviews- Greet new admits- Collaborate with team for completion of baseline care planComplete and follow up on issues and investigations as needed and communicate timely with DON and Administrator as needed.
