Main duties of the job
The role will require:
* The ability to communicate effectively with others at all levels.
* Working flexibly to provide service delivery across the local area (the service operates from 7:30 – 18:30 Mon‑Fri).

* Working autonomously and managing your own caseload whilst supervising others and working within a multidisciplinary team.

* Experience of working in a fast paced and varied environment.
* Maintaining contemporaneous clinical notes in accordance with Trust guidelines.
* Traveling to deliver care services.
* Coordinating caseload, triaging referrals, allocating workload, prioritising urgent response assessments alongside colleagues; performing holistic, goal‑focused, patient‑centred assessments and developing thorough treatment programmes to meet goals; supported to manage a varied caseload spanning all occupational therapy specialities.
Job responsibilities
* Work as an autonomous practitioner within an integrated team providing timely and expert professional assessment and treatment interventions.
* Provide case management for a designated caseload of patients with immediate and/or complex care needs.
* Triage patients to ensure prioritisation of clinical need; develop and coordinate individualised rehabilitation plans in close liaison with the team, the patient, and their carers.
* Be responsible for the timely assessment and treatment of patients with urgent and/or complex Occupational Therapy needs.
* Coordinate appropriate interdisciplinary, patient‑centred care that promotes independence for patients on their caseload.
* Undertake occupational therapy assessments for a designated caseload, addressing occupational performance and skill deficits, enabling the client in areas of self‑maintenance, productivity, and leisure.
* Assess clients in relation to equipment and specialist equipment needs for minor adaptations (e.g., rails, half steps), following the process to refer for major adaptations.
* Assess and manage caseload for Intensive Support patients which includes Pathway 1 and Admission Prevention patients.
Person Specification
Additional Criteria
* Ability to meet the travel requirements of the role.
Experience
* Application of the Occupational Therapy process.
* Use of functional assessment and activity analysis.
* Experience of working in a community setting.
Knowledge
* Knowledge of current best practice in community occupational therapy.
* Understanding of NHS health legislation, current practice and risk assessment and management.
Qualifications
* Diploma/degree in Occupational Therapy.
* Health Professionals Council registration.
* Original certificates will be required to verify qualifications at the point of offer.
Skills & Abilities
* IT skills and competence in using a computer.
