About NPC
Northumbria Primary Care Ltd (NPC) is a pioneering partnership of local GPs operating eight general practices across Northumberland and North Tyneside, with around 50,000 registered patients. NPC is a wholly owned subsidiary of Northumbria Healthcare NHS Foundation Trust.
Role Overview
You will work as an Advanced Clinical Practitioner (ACP) in a dynamic GP practice, delivering high‐quality, patient‑centred care across face‑to‑face and telephone consultations, as well as supporting care homes and home visits where required. Flexibility in working hours and locations is offered, with a focus on maintaining safe and effective patient care.
Key Responsibilities
* Assess and deliver evidence‑based care for patients with acute and chronic conditions, using autonomous clinical decision‑making.
* Triage patients effectively, offering telephone advice/support and arranging face‑to‑face appointments with appropriate clinicians.
* Provide autonomous critical thinking and diagnostic reasoning; manage patient care over time and organise necessary services.
* Support the medical and nursing teams, contribute to the practice’s clinical rota, and attend on‑call sessions.
* Act as a leader and role model: supervise junior team members, mentor, coach, and supervise nursing staff.
* Deliver comprehensive clinical assessments, initiate, evaluate and modify appropriate interventions.
* Recognise and address risks related to Third‑Party Artificial Intelligence (AI) in the recruitment and selection process.
Qualifications
* Valid UK driving licence and ownership of a car for work or a Trust personal lease vehicle.
* Autonomous practitioner experience, capable of independent decision‑making and complex patient management.
* Strong leadership, communication and teaching skills to support a multidisciplinary team.
* Familiarity with GP practice workflows and the NHS clinical governance framework.
* Experience with care‑home and home‑visit provisions preferred.
* Fit‑and‑Proper Person Requirements (FPPR) checks will be undertaken for all successful applicants.
