Job Overview

An opportunity has become available in the North D2A bed service for a permanent level 1/Band 5 Social Worker. We need a passionate, resilient and dynamic individual to join this knowledgeable and enthusiastic team.

The work is varied and challenging, focusing on pathway 2 of the D2A service. You will be supporting and assessing people within a rehab focused bed-based setting following an acute hospital stay. The core work will be undertaking statutory assessments in complex situations, so knowledge of Mental Capacity and Continuing Health Care legislation is crucial.

Main duties of the job

The successful applicant will be part of a multi-disciplinary team including nurses, physiotherapists, occupational therapists, discharge facilitators and support staff within the Haywood hospital and private residential and nursing homes. The team also works closely with a number of external partners such as the ICB and Staffordshire County Council. The work is on a short-term basis and cases are usually closed within 12 weeks; this means the successful individual will need to be able to work at pace and be able to move effectively between competing priorities.

We are looking for a social worker that wants to develop their skills and progress. We will support the right individual to progress to level 2 social worker when the ASYE is completed, provided they are able to demonstrate B6 capability.
