Responsibilities
* Support residents to remain independent in their own homes with access to information and services that promote a strength-based approach.
* Work in a multi-disciplinary team with Social Workers and Occupational Therapists; collaborate with the Live Well Hub (Community Navigators, volunteers and community services) to help people meet their outcomes.
* Demonstrate excellent communication and assessment skills and enjoy working in a fast-paced but supportive environment.
* Some weekend working is required to meet service requirements (5 days over 7).
Qualifications & Experience
* Experience of working within Adult Social Care or demonstrate a passion to do so.
* A valid driving licence and access to a car is essential for this role.
* You must be fluent in English (as a requirement of Part 7 of the Immigration Act for the effective performance of a customer-facing role).
