Would you like to join us in supporting those effected by drug and alcohol issues to improve their health and wellbeing?
The Role
The Health and Wellbeing Practitioner will work to coach awareness of unhealthy habits and associated risks, provide advice and guidance on how to change behaviours. It will include a manualised short (no longer than 6 to 8 weeks) program of interventions and encourage and signpost to the local services when appropriate.
This role is an outreach post, and the work will include evenings and weekends, if needed, to fulfil business needs. For more information, please refer to the Job Description.
The main purpose for the role is:
* To develop and deliver a specialist wellbeing function in the community for low complexity and low dependency substance users. Offering advice, support, guidance and coaching to promote positive future behaviours, including:
o proactive and assertive assessment and engagement activities,
o education on the impact of substances on health and wellbeing,
o explore behaviours and identify methods to make positive change,
o planning for changing and maintaining future healthy behaviours.
* To deliver interventions in a variety of settings including GP practices and community centres, in one-to-one, face-to-face, digital and remote ways, at a range of times including evenings and weekends, to provide a wide range of accessibility options and reduce barriers to maximise engagement.
* To provide a highly visible point of contact to promote the service, working proactively and collaboratively with colleagues and all stakeholders, both internally and externally.
