The Integrated Family Healthcare GP Federation islooking for a compassionate and dedicated ADHD Service Navigator to join ourteam working on our new ADHD pilot project in Hastings. Thisnew service will offer an alternative for patients age 18+ awaiting a diagnosisof ADHD, under the right to choose pathway.
Main duties of the job
The Service Navigator will have a key role insupporting our patients through the patient pathway. We are looking for ahighly organised individual who is confident in managing patient workflow andhas strong interpersonal skills to engage effectively with patients. The carecoordinator will need to ensure smooth booking of appointments, completion ofquestionnaires and assessment forms, and communication between the patient andthe service.
You will be working within a small, supportive teamwithin a new pilot project in Hastings.This role is primarily office based to enable closecoordination with clinicians and real-time pathway coordination and management.Flexible working may be considered once establishedin the role and subject to the service needs.Induction and training provided.

The Integrated Family Healthcare GP Federation's primary mission is to support the delivery of the Directed Enhanced Service (DES) through the Hastings & St Leonards Primary Care Network (PCN).
We comprise of 8 general practice surgeries and medical centres across Hastings and St Leonards, serving a population of 100,000 patients.
Job responsibilities
Key Responsibilities
Actas a point of contact within defined administrative boundaries.
Provideclear explanations of the pathway process and schedule appointments
Supportpatients to complete required forms or documentation where necessary.
Signpostpatients into the wrap around care.
Pathway& Systems Management
Actas the central coordinator for the ADHD pathway, maintaining oversight of allpatients progressing through assessment, treatment and follow-up.
Managementof clinic booking systems, ensuring clinics are fully utilised andappropriately scheduled.
Monitorpatient progression and proactively identify gaps, delays or restrictions inthe pathway.
Maintainaccurate tracking systems to support capacity planning and reporting.
Escalateoperational risks or delays to the Service Delivery Manager in a timely manner.
Takethe lead in resolving day to day workflow issues to ensure of continuity of theservice.
Supportrota planning in the clinical systems.
Data& Information Governance
Ensureaccurate and timely entry of patient information across the relevant clinicalsystems.
Maintainhigh standards of data quality and record accuracy.
Supportreporting and auditing requirements.
Handleconfidential patient information in line with data protection and informationgovernance policies.
Administrative& Service Support
Coordinateregular MDT meetings with the clinical team and support clinical governance.
Providestructured administrative support to clinicians, ensuring documentation iscomplete and up to date.
Supportservice mobilisation and ongoing process refinement within the pilot.
Contributeto development of standard operating procedures and workflow documentation.
Person Specification
Experience
* Strong interpersonal and communication skills to engage effectively with patients, families, carers and wider healthcare teams
* Excellent organisational and time management skills with proven ability to prioritise complex workflow effectively.
* Empathetic and compassionate approach towards patients and their families
* Ability to work independently and as part of a collaborative team.
* Ability to monitor, track and report on process performance.
* Strong communication skills and ability to work effectively within a multi-disciplinary team.
* Proactive work style with the ability to solve problems autonomously.
* Ability to recognise and respond to different communication needs, adapting where necessary to support effective engagement with patients.
* Experience in working in a healthcare setting.
* Some knowledge or experience with neurodiversity, including ADHD or Autism.
* Knowledge of SystmOne or other NHS systems (training will be provided)
* Experience in supporting service improvement.
* Ability to work and adjust communication to meet different patient needs.
