Job Title: Hospital Discharge Social WorkerHourly Rate: £26.26Location: [Hospital Location in Darlington]Job Summary:We are seeking a highly motivated and experienced Social Worker to join our dedicated Hospital Discharge Team in Darlington. You will play a pivotal role in facilitating timely and safe discharges for adult patients, working collaboratively with a multi-disciplinary team to ensure patients receive the right support and care to transition back to their homes or alternative care settings.Key Responsibilities: Conduct comprehensive assessments of patient's needs, including physical, emotional, and social factors. Develop and implement person-centred discharge plans that meet individual needs. Liaise effectively with medical staff, nurses, occupational therapists, and other health care professionals. Work collaboratively with community services, including social care teams, housing support, and other relevant agencies. Advocate for patient's rights and ensure their voices are heard. Facilitate timely and safe hospital discharges, minimising delays and risks. Support patients and their families in their transition back to the community. Maintain accurate and up-to-date case records. Contribute to the ongoing development and improvement of the team's services. Comply with all relevant legislation and professional guidelines.Essential Criteria: A recognised Social Work qualification Current registration with the Health and Care Professions Council Significant post-qualifying experience in adult social work. Experience working within a hospital or health-care setting is highly desirable. Excellent communication, assessment, and care planning skills. Ability to work independently and as part of a multi-disciplinary team.* Full UK driving licence and access to a vehicle.Desirable Criteria: Experience working with specific patient populations (e.g., older adults, people with mental health conditions). Knowledge of relevant legislation and policy frameworks (e.g., Mental Capacity Act).* Experience with case management software.Benefits:* Competitive hourly rate of £26.26.Apply
