Health Care Support Worker - Overnight Nursing (ONN)
Join our team and contribute to overnight community care. This role is based in Devizes, working 20 hours and 30 minutes per week, with shift times from 20:15 to 07:45.
At HCRG Care Group, we are expanding our community services to transform out-of-hospital care. We are extending our nursing services 24/7 across Wiltshire to provide high-quality care around the clock.
If you thrive in a community setting and are looking for a new challenge, you will work alongside experienced nurses, delivering effective interventions in homes and community settings, helping prevent hospital admissions and responding swiftly to urgent referrals.
Please note: We are unable to offer overseas sponsorship for this role. If not already completed, a Care Certificate must be completed within 3 months of employment. A UK driving licence and appropriate business vehicle insurance are required for this role.
Main duties of the job
To implement treatment by following the care plan and keep the patient at the centre of their care, ensuring patient and carer participation in decision making. To undertake the daily care and support of delegated patients and deliver treatment/care within a community setting as specified in a written care plan, under the guidance of a registered practitioner. To help avoid hospital admissions and support patients to receive care in the most appropriate setting. To monitor day-to-day care of patients and support the taking of medication under the authorisation of a qualified practitioner. To maintain infection control to reduce the risk of healthcare-acquired infections and to enable patients to reach their optimum level of independence. To support terminally ill patients and their carers/relatives. To communicate with individuals about routine activities and overcome difficulties (e.g., hearing or language barriers). To document the needs and wishes of the individual and work within a multi-professional team.

* Experience of working under distributed supervision
* Experience of supporting other workers

* Physical ability to manually handle patients and use lifting aids
* Undertake specific nursing/therapy skills
* Current full driving licence and use of a car during working hours
