Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

We are looking for an experienced and motivated Band 7 PCN Lead to join our community nursing team, supporting high‑quality care for housebound patients in their own homes. As a Clinical Nurse Specialist, you will provide strong clinical leadership while overseeing the day‑to‑day management of staff and ensuring the delivery of safe, effective and compassionate care across your teams.

In this senior role, you will use your advanced community nursing expertise to offer sound clinical judgement, act as a role model and support colleagues in managing complex caseloads. You will be responsible for triaging new referrals, coordinating care delivery and allocating work to team members with the appropriate skills and competencies. As the senior clinician, you will lead on problem‑solving, decision‑making and planning care for patients with complex needs.

You will also take responsibility for workforce planning and rota management, ensuring staffing levels meet service demand.

Main duties of the job

Our Community Nursing service provides holistic assessments, personalised care planning and a wide range of clinical interventions to support housebound adults within the local area. As a Band 7 PCN Lead, you will ensure high‑quality, patient‑centred care while promoting independence and supporting patients to manage complex long‑term conditions safely at home. Strong clinical skills and a solid understanding of complex disease management are essential at this level.

You will play a key leadership role, supporting and developing your workforce to meet the demands of a busy service. This includes managing day‑to‑day staff issues, promoting wellbeing, and ensuring your team has the skills, confidence and support needed to deliver safe and effective care. Working collaboratively with Primary Care Networks and the wider MDT, you will help coordinate shared care approaches that meet the health and social care needs of the local population.

As a role model for the profession, you will actively contribute to service improvement projects, maintaining a strong focus on patient safety, experience and continuous development. The Community Nursing service operates seven days a week, including bank holidays and weekends, with core shifts between 08:00 and 18:00. Some flexibility may be considered.

Person specification

Qualifications, training and experience

Essential criteria

1. You’ll need to have a Level 6 Degree or equivalent qualification in relevant subject Nursing, Physiotherapy, Occupational Therapy or Speech and Language Therapy
2. Proven ability to work effectively within your area of practice, supported by relevant experience. As well as managing complex caseloads and supporting/ mentoring others
3. You will be a registered health care professional with the Nursing and Midwifery Council or Health and Care Professionals
4. A full, valid UK driving licence or a full driving licence issued by an EU country

Knowledge and skills

Essential criteria

5. You will need to have a customer focussed approach
6. Provide patients, clients and their families with the best possible experience
7. You will need to be able to be able to use IT systems and databases
8. You will understand where to signpost clients and families to alternative services to support behaviour change and improve health
9. You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks. You will fully understand the role and remit of a health care worker

Desirable criteria

10. Having an awareness of developments within the NHS would be an advantage as well as an understanding of National Service Frameworks and NICE guidance
