Job Title: Social Worker – Older Persons and Physical Disability
Location: Southwark (Hybrid)
Pay Rate: £33.76 per hour
Liquid Personnel is seeking a compassionate and dedicated Older Persons and Physical Disability Social Worker to join the team. The successful candidate will provide support, advocacy and care coordination for older adults and individuals with physical disabilities, helping them to achieve their full potential and improve their quality of life.
Responsibilities :
* Conduct comprehensive assessments of older adults and individuals with physical disabilities to identify their needs and develop personalized care plans.
* Work closely with healthcare professionals, caregivers, and other social services to coordinate care and support for older adults and individuals with physical disabilities.
* Respond to crises and provide immediate support and intervention to individuals and families in need.

Qualifications and Experience :
To be successful in this role, you must be registered with Social Work England, be eligible to work in the UK, and hold a full UK driving licence. You should also possess a degree-level qualification or equivalent in Social Work, along with relevant post-qualified experience.
