We are seeking a compassionate and dedicated individual, with a keen interest in frailty, to ensure we are promoting independence, preventing deconditioning and assisting with daily activities to support in the patient's home. The ability to demonstrate care and compassion to patients and their relatives is essential. You will need to have a caring and professional approach to patient care. The ability to work as part of the multidisciplinary team, to ensure high standards of care is delivered to the patients. The ability to speak Welsh is desirable for this post, however we welcome applications in English or Welsh.
