About The Team
The Housing Solutions team assess residents across Hertfordshire in their homes for major housing adaptations. The team has a blend of community based Occupational Therapists and Community Care Officer Trusted Assessors who complete specialist housing needs assessments to identify major adaptation solutions to enable a person to have better access to the facilities within their homes. We explore personal strengths, their family and friends' support networks, and using expert knowledge we connect people with interventions that will help individuals and families to get on with their lives, achieving prevention, reducing or delaying the need for statutory provision of care services.

Housing Solutions is a county wide service and whilst we patch work within geographical areas, we may at times ask the team for flexibility to cover a wider area depending on the needs of the service. We provide training, mentoring and regular supervision from a supportive team.

About The Role
Trusted assessors will
:

* Conduct holistic, person-centred assessments during home visits.
* Recommend both minor and major housing adaptations.
* Possess a thorough understanding of The Care Act and Housing Legislation.
* Connect individuals with local community resources.
* Operate in a preventative and enabling manner, fostering independence within homes and communities.
* Build strong relationships with district and borough council adaptations teams.
* Work on their own initiative as well as working as part of a team
* As part of your probation period, you will complete a Level 4 Trusted Assessor accreditation.

About You
Essential:

* Full driving licence
* Understanding of The Care Act and Housing Legislation
* Experience working in a social care setting

Desirable:

* Previous experience of minor or major home adaptations.
* Completed a functional assessment course or have experience in equipment provision or other relevant training (i.e. NHS).
