Surrey Downs Health & Care The Rehabilitation Support Worker role will function as part of the new and exciting Recovery At Home team by providing care, support and rehabilitation within a community setting to provide a seamless service and to support discharges from hospital or prevent admission to hospital. The post holder will undertake a range of delegated tasks, following a care plan and will report to the Team Lead. This role will be supported to develop skills and abilities to work with this client group to provide the appropriate level of care. We are looking for dynamic and caring individuals who will play a key role in driving our Discharge to Assess services in Surrey Downs. You will have excellent organisational and time management skills and be passionate about delivering holistic care that has the patient at its centre. You will relish developing new ways of working to improve quality and develop a “one team” culture. The service provides a 7 day / week service from 8am to 8pm and staff are required to work flexibly over the full week. We are open to conversations about flexibility with shift patterns. Home First supports the approach of “why not home, why not today?” Wherever possible patients should be supported home for ongoing assessments where this is not possible alternative pathways are available. Staying in hospital for longer than necessary has a negative impact on outcomes and ensuring patients are given the chance to continue their lives at home is vital for their long-term wellbeing. @home works across organisational boundaries and as an integral part of the integrated health and care team is responsible for providing rapid access, enhanced, short term health and care support for people who would otherwise need emergency hospital admission as well for as those medically stable to return home from an acute hospital but requiring enhanced support as one stage in their on-going care pathway. The @home service spans the acute and community providing joined up, co-ordinated care to local citizens with health and social care needs. The Recovery Athome Team will form an integral and essential part of Discharge to Assess, providing timely care and support to facilitate discharge from hospital and onward assessment and identification of need. The team will work closely with health and social care services It is anticipated that this role will continue to develop through the acquisition of further skills, knowledge and competencies supported by the team Surrey Downs Health and Care deliver care closer to people’s own communities through our Primary Care Networks, Community Hospitals, Specialist Services and our innovative partnership of local NHS organisations. Surrey Downs Health And Care Has a Track Record Of Providing Person Centered Care That Goes Beyond Organisational Boundaries To Do What Is Best For The Individual. This Partnership Includes The three GP federations GP Health Partners, Dorking Health Care and Surrey Medical Network representing practices that operate in the Surrey Downs area Epsom and St Helier’s University Hospitals NHS Trust Surrey Council County Historically, there have been boundary lines between the organisations that provide care to people in their homes, in GP surgeries and in hospitals, but we have always been united in our mission to provide great care to the people who need us. It’s on those grounds that the Surrey Downs Health and Care was formed – we want local people to receive the care that they need in the right environment.
