Job overview
We are recruiting an enthusiastic Band 3 Community Support Worker to join our Community Response Team covering South Warwickshire, with bases at Stratford‑upon‑Avon and Leamington Spa. The service aims to provide high quality care and support patients to remain independent within their own home and to facilitate safe discharges from hospital.
Main duties of the job
Key responsibilities
The Band 3 Clinical Support Worker will undertake high quality, caring and compassionate personal care and associated duties under the direction of registered staff as part of the multidisciplinary team. You will support patients in the community and work closely with social services, GPs, community nursing teams, patients, relatives and carers.
Shifts are allocated pro rata and the team works 08:30‑21:30, 7 days a week, including weekends and bank holidays. You will provide support to patients to remain independent within their own home and to facilitate safe discharges from hospital.

Detailed job description and main responsibilities
Professional Values

Clinical Practice
Accurately and promptly carry out instructions of the registered professional. Assist in the supervision of other healthcare and support staff as directed. Carry out duties specified within the Trust’s approved core task list for Clinical Support Workers/Healthcare Assistants, without direct supervision. Report any patient concerns or observed changes in condition. Provide and deliver a high standard of individualised care. Document all care accurately and maintain confidentiality. Assist in the preparation of patients for treatment, investigations or procedures. Keep the clinical area clean, tidy and safe, and adhere to infection control procedures. Chaperone or escort patients as needed. Support relatives under the guidance of a registered professional. Assist with the discharge process as directed.
Communications and Interpersonal Relationships
Welcome patients, relatives and visitors to the clinical area. Introduce yourself in line with Trust privacy and dignity promises. Answer the telephone politely and pass on enquiries. Communicate effectively within the multidisciplinary team and other departments. Contribute to reporting, handover and documentation of patient care. Report incidents, accidents and complaints as per Trust policy.
Team Working
Ensure safe keeping of patient’s property. Promote a professional, welcoming environment. Manage equipment and cleaning responsibilities. Ensure waste and soiled linen is disposed of appropriately. Participate in audit as directed. Support housekeeping duties as necessary.
Education, Training and Personal Development
Attend mandatory training sessions (infection control, resuscitation, manual handling). Complete the Clinical Support Worker/Healthcare Assistant Induction programme. Participate in appraisals and personal reviews. Undertake training as necessary for the development of the post and keep up to date knowledge of changes in policies and procedures.

Qualifications
* GCSE level A–C English and Maths or equivalent
* NVQ/Diploma Level 2 in healthcare, willing to work towards Level 3 or equivalent higher apprenticeship
* NVQ/Diploma Level 3 in healthcare or equivalent higher apprenticeship
Experience
* Experience delivering care in a healthcare or people‑centred environment
* Experience working as part of a team

* Evidence of competence in care skills
