There will be some weekend working with Saturdays and Sundays covered on a rota basis.
Main duties of the job
8. Act as a first point of assistance for both internal and external enquiries relating to patient discharge
9. Facilitate and support effective and early communication between hospital staff, the patient and their family with regards to discharge planning.
10. Manage patient communications via telephone and through Trust systems to create letters and emails
11. To act as a point of contact for queries from clinical and non-clinical staff
12. Liaise with clinical and non-clinical teams across the Trust via telephone and email
13. Escalate patient queries, capacity problems and general issues to line manager or relevant clinical staff as appropriate
The position is full time, Mon-Fri 8am-4pm with weekend working on a rota.
