The Sleep Medicine Case Management Officer assesses, plans, coordinates, and monitors care for patients with sleep‑disordered breathing. The role ensures quality, continuity, and effective use of healthcare resources while working closely with multidisciplinary teams to optimize patient outcomes.
Job Scope
* Assess patients’ medical, psychological, social, environmental, vocational, and financial needs.
* Develop and implement individualized care plans in collaboration with clinicians and healthcare professionals.
* Assist in developing and reviewing clinical guidelines, standards, and procedures.
* Participate in research studies and quality improvement initiatives.
* Conduct training and education sessions for healthcare staff on sleep medicine.
* Provide patient education on self‑management of sleep‑disordered breathing.
* Monitor patients’ progress through regular telephone follow‑ups.
* Perform patient assessments, including vital signs and symptom monitoring.
* Recruit suitable patients for disease management programme and services.
* Participate in quality control and departmental projects.
* Coordinate step‑down care and follow‑up appointments according to protocols.
* Ad hoc duties as assigned by supervisor.
Job Requirements
* Bachelor Degree in Medical Technology, Health Science, Biomedical Science or equivalent.
* Experience in sleep medicine preferred; CPSGT/RPSGT certification is a plus.
* Basic Cardiac Life Support (BCLS) certification.
* Strong interpersonal, communication, analytical, and organizational skills.
