Responsibilities
* Ongoing assessment of care needs and development, implementation and evaluation of programmes of care.
* Carry out all relevant forms of care and take responsibility for caseload holders in absence of the post holder.
* Manage work area, clinical caseloads, including supervision and development of staff.
Qualifications
* Band 6 Senior community staff Nurse.
* Effective communication and negotiation skills.
* Advocacy for patients with the ability to inspire people at all levels.
* Understanding of the expanding role of community nursing and commitment to advance it.
* Sound understanding of the current primary care agenda.
* Excellent communication skills and effective leadership and decision‑making skills.
* Innovative, able to work autonomously while promoting a positive, supportive learning environment for team members and students.
* Interest in developing leadership skills in supporting the team leader.
