We are seeking an experienced Registered Nurse to join the Vulnerabilities team with a passion for, and experience in, providing and championing patient-centred Dementia care. The role involves supporting people with Dementia and their families who have complex needs, offering specialist support, guidance, and practical solutions.
The successful candidate will support the implementation of the Trust Dementia Standards, working collaboratively with Clinical Delivery Groups and staff to meet the health needs of people with Dementia.
You will also be responsible for supporting, coordinating, and delivering education and training to staff, enhancing their understanding of Dementia, its progression, behavioural changes, and communication. This includes evaluating and auditing care, supporting practice development, and quality improvements.
This role requires a flexible approach, with travel across sites. In partnership with the multidisciplinary team, you will develop, implement, and evaluate a seamless Dementia specialist service, ensuring patients receive the highest standard of clinical care.
As a nursing expert, you will demonstrate a high level of expertise in Dementia, acting as a role model, providing advice, education, and support to staff, patients, families, and carers.
You will support clinical audit activities for Dementia, contribute to the development of clinical practice and standards, and help develop policies, procedures, protocols, and guidelines.
You will be accountable for safe and effective clinical practice, supporting team performance, and ensuring the efficient use of resources. You will provide educational and training packages for healthcare staff and patients.
This role involves working Trust-wide and across all sites, sharing expertise across multidisciplinary teams. Excellent communication, organizational, and interpersonal skills are essential. You should be able to work independently or as part of a team, be resilient to changing workloads, prioritize tasks, and deliver high standards of care.
