Overview

This is a senior Nursing/Midwifery role within the organisation and there is an expectation that the post holder will fully contribute to the leadership and delivery of high quality patient care within the care group. To deliver a comprehensive and specialised nursing service to patients and their carers with non-site-specific presentations. To ensure timely, coordinated, and compassionate care throughout the diagnostic pathway for patients referred by primary care providers with non-specific symptoms suggestive of cancer. The post-holder is responsible for supporting the continuous improvement of quality services and safeguarding high standards of care by sustaining an environment in which excellence in clinical care can flourish.

Responsibilities

* Triage, prioritise, and coordinate referrals in line with national and local pathways, ensuring patients are assessed within the two-week-wait target.
* Deliver a high-quality, specialist nursing service, providing expert clinical advice, information, and support to patients and their carers throughout the diagnostic and care pathway.
* Undertake comprehensive patient assessments, identifying physical, psychological, and social needs, and initiate appropriate interventions and onward referrals.
* Act as a key worker and central point of contact for patients, coordinating care and liaising effectively with primary care, secondary care, and multidisciplinary teams (MDT).
* Support and empower patients to make informed decisions regarding investigations and treatment options through the application of specialist knowledge and evidence-based practice.
* Ensure patients are fully informed of investigation results and agree appropriate next steps, including referral to specialist teams or safe and supported discharge back to primary care.
* Facilitate timely referrals to relevant services, including dietetics, specialist nursing services, allied health professionals, voluntary organisations, and patient support groups, as required.
* Participate in MDT meetings and other relevant forums, contributing specialist nursing expertise and acting as a patient advocate to ensure patient-centred care.
