Working Within GP Surgery Settings Across Medway, You Will
* Provide holistic support to individuals with Serious Mental Illness (SMI) and dementia, helping them remain stable within the community
* Carry out annual reviews and physical health checks
* Promote recovery by signposting patients and carers to relevant community and voluntary services
* Collaborate closely with local mental health teams and the KMPT Primary Care Mental Health Service to deliver joined-up, efficient care
* Participate in multi-disciplinary team (MDT) meetings within the Primary Care Network

* MCH encourages all staff to be double COVID vaccinated to protect themselves, their colleagues, their family and their patients.
* MCH is considering merging with Kent Community Health Foundation Trust (KCHFT). If the merger goes ahead, the proposed transfer date is 30th September 2026 and this role will be subject to TUPE transfer to KCHFT.
