An exciting opportunity has arisen for a Nursery Nurse to join our dedicated Community Health Visiting Team in Wrightington, Wigan and Leigh Foundation Trust. A requirement of the role will be to support families by assessing development, utilising appropriate assessment tools and delivering packages of care under the direction of the named Health Visitor.
You will be required to work in a variety of settings such as the family home and community hubs including Children Centres and Early Years settings and will need to be able to travel between these sites independently. You will be required to work independently and as part of a multi disciplinary team. You will be required to deliver targeted interventions inline with the public health Agenda and the Healthy Child Programme. You will be expected to deliver health promotion strategies on a one to one basis and in group settings.
On offer is a permanent part-time post (25.45 hours) working across Monday to Friday.
Professional childcare knowledge is required eg BTEC / NNEB or equivalent. A working knowledge of SystmOne is preferable although training will be provided.

Main duties of the job
The role will include carrying out children’s development reviews, group activities, Early Help meetings, health promotion and health education programmes as delegated by the Health Visitor in the provision of the Healthy Child Programme.
You will do so in line with the local policy and National Guidance, offering early support as required. You will work with all families including universal, universal plus and universal partnership plus and you will be expected to actively participate/lead when working with families who need Early Helps to support them. You must be able to travel across Trust independently.
You will be expected to work and liaise with our partner agencies including nurseries, Startwell services, General practitioners and schools.
Please note: Candidates will be asked to provide original qualifications at interview. An Enhanced DBS (with checks against the Adults and Children's Barred lists) will be obtained for the successful candidate at pre-employment check stage.
The role will require you to work across all areas in the Trust which may include working with patients designated as positive, asymptomatic and negative for COVID-19.

Detailed job description and main responsibilities
Key results from the job holder
Delivering the Healthy Child Programme
1. Arrange and undertake child health development assessments and reviews in a variety of settings to suit the needs of the child, young person and family.
2. Undertake home visits to families delegated by the Health Visitor/PHCN to provide extra specific support as part of a planned programme of care.
3. Provide appropriate, evidenced based advice to families, in a variety of settings, on a range of topics/issues.
4. Provide appropriate advice to families, refer to other agencies and signpost families if appropriate.
5. Refer the child/families to other disciplines, professions, or agencies when appropriate to do so – informing and discussing with the Health Visitor/ PHCN.
6. Support the organisation and delivery of drop-ins where the Community Nursery Nurse is deemed by the Health Visitor / PHCN to be to be the most suitable person.
7. Provide timely feedback to the Health Visitor/ PHCN on work carried out.
8. Report to the appropriate Health Visitor/PHCN when there are any concerns about a child (this includes; physical/emotional or developmental condition of child, environmental and safeguarding concerns etc.)
9. Adhere to Local Safeguarding policy and procedure to enable relevant referrals to be made to Children’s Social Care.

11. Support delivery of all national screening programmes undertaken by the 0-19 Service (e.g. NCMP)
Health Promotion
12. Lead Health Promotion activities in a variety of settings
13. Provide Public Health Brief Interventions as identified
14. Set up and run groups relevant to the communities needs and according to individual Public Health Assistant Practitioners skills, (including carrying out risk assessments of premises etc.
15. Prepare health education displays.
16. Participate in presentation of health promotion activities to general public e.g. market stalls.
17. Keep health education materials/leaflets in good order, ensuring adequate supplies, and ordering as necessary.
18. Ensure information is evidence based.
Organisation of Work
19. Organise own workload effectively, ensuring a good use of time and an efficient and timely service to Children young people and families.
20. Submit accurate performance data on a monthly basis and in a timely manner
Record Management
21. Complete Health records paper or electronic, in compliance with policy
22. Maintain strict confidentiality for all clients.
23. Maintain record keeping skills and knowledge by undertaking regular training.
Responsibility for Health & Safety
Compliance with the Health & Safety at Work Act 1974 – the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
Supervision and Development
24. Attend for 1:1 supervision with the Team Leader or Named Practitioner.
25. Attend and partake in group clinical supervision.
26. Participate and engage with Personal Development Reviews identifying any training needs and ensuring they are met.
27. Identify training/updates required to maintain own clinical competence.
28. Acknowledge own limitations and work within the confines of the remit of Public Health Assistant Practitioner and the knowledge and skills framework
29. Adhere to care pathways used with the service.
30. Attend relevant in-service training and other professional seminars, lectures, courses, and study days.
31. Complete mandatory training requirements annually.
32. Assist in the education of learners.
Work Circumstances & on-call
Various hours available (Monday to Friday) with no on-call responsibilities.
