Job overview

There is currently an exciting opportunity for a Band 6 to join the Urgent Community Response and Hospital at Home in Hambleton & Richmondshire (H&R). You will work within a large multidisciplinary team offering specialist Physiotherapy assessment and Therapy intervention.

You will need to be able to work as part of a team and on your own, frequently using your own initiative. Excellent communication skills, organisational skills, enthusiasm, commitment and ability to negotiate across a wide range of agencies are also vital in this post.

Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.

Main duties of the job

The role will require being involved with clients with a range of multiple medical, social and emotional problems, as well as a substantial amount of home visits. The ability to travel independently is therefore an essential part of this post.

The team currently provides a 7 day service so there is an expectation for the successful candidate to be included in the cover for weekend working.

The service aims to enable individuals to maximise their independence at home and to support timely discharge and prevent avoidable admission to hospital or long term care.

Qualifications and training

Essential criteria

1. To hold a Diploma or Degree awarded by professional body
2. Registered as fit to practice by professional body, Health and Care Professions Council (HCPC)
3. To have completed the physical assessment and disease management skills course (level 6) or be working towards this within the first 12 months.
4. Teaching and mentoring qualification
5. Professional knowledge acquired through degree supplemented by post graduate diploma specialist training, experience, short courses plus further specialist training to degree equivalent level
6. Highly developed specialist knowledge, underpinned by theory and experience

Kowledge and Skills

Essential criteria

7. Demonstrates an ability to work on own initiative as well as part of a team
8. Able to assume additional managerial responsibility and prioritise this accordingly
9. Demonstrates competent IT and system skills
10. Effective people management skills
11. Participate in regular clinical supervision
12. Knowledge of clinical governance and risk assessment/management

Experience

Essential criteria

13. To display effective communication skills, communicating complex, sensitive and contentious information to others
14. Demonstrates an ability to prioritise and co-ordinate the workload of the team to ensure fairness and equality
15. Demonstrates excellent organisational/ delegation/ prioritisation skills.
16. Demonstrates evidence of teaching skills to a range of professionals
