Typical day as a Support Coordinator Work with a team of Support Coordinators to deliver a high-quality service that meets Home Groups KPIs. Support customers to build independence and achieve their goals under the guidance of your Senior Client Service Manager. Assist customers with varied needs including mental health, housing, and complex behaviours. Create and update support plans and risk assessments, helping customers with daily tasks like cooking, budgeting, and moving to independent living. Ensure properties are clean, safe, and welcoming, helping reduce voids. You are Experienced in creating person-centred support plans, risk assessments, and reviews. Skilled in coordinating and assessing customer referrals. Empathetic and passionate about supporting customers in housing or care settings. Youll also need a car insured for business use and we pay for the mileage. Job details 24/7 service with a 4-week rolling rota (including weekends and bank holidays, no night shifts). Occasional flexibility required, and well offer flexibility in return. Some lone working during morning or late shifts. Basic Microsoft Word and Excel skills required. Confident using technology for support plans, training, and collaboration.
