Prioritise patients whorequire high level of input and complete a review and monitoring of thesepatients. Any issues identified arediscussed with the pharmacist and/or the medical team, who will review andamend the prescription as appropriate. Follow up on-goingmonitoring of chronic conditions and discuss outcomes with the pharmacistand/or the medical team for review. Provide patient counsellingfor all patients admitted to our wards, including on-going medicines and newlystarted medicines. Carry out clinicalprioritisation duties that require good analytical and decision-making skills,combined with holistic clinical knowledge.
