Overview
We have exciting opportunities for experienced Band 5 community nurses to join our Out of Hours Team. If you are experienced, enthusiastic, motivated and compassionate we would love to hear from you. The service is open seven days a week, various hours available. As an organisation, we are presently reviewing the delivery of 24-hour nursing care. Any successful applicants will be part of a future service consultation that takes place. Shortlisting for this vacancy is due to commence on 12/05/2026 and the proposed interview date is 01/06/2026. We cover the whole of the West Cheshire geographical footprint liaising closely with the eight care community teams and their caseload. The Community Nursing OOHs base sits within the Countess of Chester Health Park. It’s important that you have access to your own transport to undertake visits. Mileage expenses are reimbursed in addition to your hourly rate.
Responsibilities
As a Community Staff Nurse the successful candidate will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach. You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.

Induction and progression
All new starters will commence in post on one fixed start date each month, in line with our Prepare to Care (P2C) induction programme date. P2C offers up to 3 weeks training to support you to meet the induction / initial training requirements of the role.
