Sandwell Metropolitan Borough Council is looking to appoint a STAR Trusted Assessor. 18.5 hours per week.
Responsibilities
* Carry out comprehensive assessments in hospital and community settings across the Borough of Sandwell.
* Identify individual needs and personal outcomes through thorough assessment.
* Develop personalised support plans aimed at meeting identified needs.
* Complete appropriate risk assessments to identify potential risks.
* Implement actions and strategies to minimise identified risks.
Essential Requirements
* Minimum of 1 year’s experience working with vulnerable adults and carers.
* Knowledge of assessment frameworks, including risk assessment.
* Ability to travel throughout the borough as required.
* Availability for 7-day working, including some unsocial hours.
* GCSE English and Maths (or equivalent qualification).
