Responsibilities
* Undertake Care Act assessments and reviews, ensuring the needs of service users are at the forefront of your work.
* Complete Mental Capacity Act evaluations and Safeguarding assessments with professional curiosity and sound judgement.
* Work seamlessly within our Integrated Neighbourhood Team, delivering comprehensive care assessments and collaborating with NHS colleagues and community health services.
* Ensure adherence to Continuing Health Care (CHC) frameworks and guidelines in your practice.
* Facilitate quality communications and interventions among teams, service users, and families for optimal care delivery and integrity in safeguarding.
