An exciting opportunity has arisen for an experienced RSW to join our Intermediate Care Team as part of the Integrated Community & Support Services (ICASS) in the St Andrews Locality.
The Intermediate Care Service provides rehabilitation to the adult population within the locality. This will be delivered to service users as in-patients, out-patients, or within their own homes.
The primary function of this role is to work with Occupational Therapy, Physiotherapy, and District Nursing Services within Intermediate Care to deliver therapeutic treatment programs to service users in the ward setting or their own homes, preventing hospital admission and supporting early discharge from hospital. These services will provide specialist assessments and allocate work to you. This post mainly involves daily care in the community, operating 365 days a year from 0800 to 2200.
You will require an SVQ Level III in Health and Social Care or equivalent. Experience working as a support worker is desirable, especially with older people, who are the main group of service users. You will work as part of a multidisciplinary team, carrying out ongoing treatment programs for a mixed caseload.
You should have good organizational skills, excellent communication skills, and the ability to work independently and within a team. Flexibility to work across different community hospital sites and 15 hours per week over 7 days is necessary.
Our service offers regular supervision, PDP, and training opportunities. The ability to travel throughout the area is essential as this is a community-based post.
