This will be a fixed term role within the Diabetes Integrated Care Service for an experienced Diabetes Specialist Nurse to assess, develop and implement programs of care providing highly specialised advice on specialist diabetes care within the Somerset PCN / ICB.
Practise in compliance with your respective code of professional conduct and within your scope of practice. Being responsible and accountable for decisions, actions, and omissions at this level of practice.
The post holder will work in line with The TREND Integrated Care and Competency Framework for Adult Diabetes Nursing to Senior Practitioner or Expert Nurse level.
Liaise with non-specialist teams in the development of management plans for patients with complex diabetes related problems including recurring hyperglycaemia, hypoglycaemia fear or unawareness, renal disease, partial sight, and diabetic foot disease to prevent admissions to the acute services and provide a safe gateway for discharge back to the community.
Provide the diabetes service within outpatient settings, including GP practices, via video or telephone consultations or via Virtual Clinics within speciality areas.

AnalyticsAutonomously provide and deliver complex care advice on duty days, triaging referrals and monitoring urgent care needs from GPs/community hospitals/district nursing services to prevent hospital admission

Expert knowledge of all types of diabetes treatments and therapies and appropriate use, dependent on acute, longer-term setting and therapeutic goals.
Lead, develop, implement, and maintain a minimum of six-monthly virtual clinics within primary care. These could be face‑to‑face or virtually for best patient care and development of services across Somerset.
Lead, develop, implement, and maintain patient group education for diabetes medication starts to support locally based patient centred care and development of services.
Prescribe medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence‑based practice and national and practice protocols, and within scope of practice.
Initiation of technology to improve Diabetes care such as continuous glucose monitoring/flash monitoring and the ongoing education and support for effective use.
