Senior Occupational Therapist Specialising in Proactive Care BMH PCN

This role is for the Beeston, Middleton and Hunslet Primary Care Network (PCN)

Primary Care Networks (PCNs) form a key building block ofthe NHS long-term plan. Bringing general practices together to work at scalehas been a policy priority for some years for a range of reasons, includingimproving the ability of practices to recruit and retain staff; to managefinancial and estates pressures; to provide a wider range of services topatients and to integrate with the wider health and care system more easily.PCNs were formally ratified in July 2019.


Main duties of the job

Beeston, Middleton & Hunslet PCN is seeking a SeniorOccupationalTherapist to be part of our multi-disciplinary and multi-agencyteam of professionals. We are looking for an individual who is passionate aboutPrimary Care and is keen to work across traditional health and social careboundaries. This is open to anyone with relevant registration. This is anexciting and unique opportunity to be at the forefront of the service deliveryof our new PCN Team, including Proactive Care Team. Working both independently,and within our multi agency team, the role will operate across the traditionalhealth and social care organisational boundaries. This includes our GPpractices and will deliver enhanced proactive care for older people withfrailty.We are looking for an Senior Occupational Therapist witha strong interest in working in Primary Care and who aspires to provide acontinuous improvement approach to delivering responsive, safe,patient-centred, and effective care within a team. The service, within whichthe post holder would work, supports patients with multifaceted health problemsliving in their own homes. The service liaises with carers, families, andcolleagues to maintain and maximise individualised patient-centred care. Italso supports General Practice in delivering exciting new developments and thiscan be flexible depending on skills, expertise, and interests.


About us

Beeston, Middleton and Hunslet PCN (BMH PCN) workcollaboratively together, this role is designed to work across both, ensuing acontinuity of care between. BMH PCN has a patient list size of circa 82,000. Wehave a significant workforce in place which brings wonderful opportunities forin-house multi-disciplinary team working. We have a well-developedmultidisciplinary team including a Pharmacy Team, Occupational Therapists,Social Prescribers, First Contact Physiotherapists, a range of nursing/ HCAstaff and more which means that the majority of any patient's needs can now beprovided on their own doorstep.

PCN Clinical Lead & Business Manager

You will be working towards the delivery of theoutcomes outlined in the Primary Care Network Directed Enhanced ServiceSpecification.

The post holder will be employed by South & EastLeeds GP Group, but will be deployed and managed day to day from within the Beeston,Middleton & Hunslet Primary Care Network

Main Tasks:

Clinical

Deliver expertknowledge, advanced clinical reasoning and care planning skills to themulti-disciplinary team.

To use advancedknowledge and practical experience to supervise, co-ordinate and manage theproactive care planning of the designated patient cohorts

To liaise with otherhealth and social care professionals and agencies to ensure holistic care isprovided and the care packages offered are appropriate and consistent withthe patients needs and aspirations.

To ensure that thepatients and their families and carers are fully involved in the assessmentand care planning process.

To ensure bestpractice relating to care planning and the management of frailty andlong-term conditions is adhered to

Managerial Responsibilities:

To co-ordinate, monitor and assess the work ofthe PCN Proactive Care Team

Provide Clinical Supervision to the PCNProactive Care Team on a regular basis

To provide emotional support for team membersin potentially stressful work situations or adverse incidents

To identify any service improvements orefficiencies

Be involved in service development

To contribute to the staff recruitment process

To complete return to work interviews withstaff following sickness in line with the Sickness Absence policy

Health and Safety - The post-holder will assist in promoting andmaintaining their own and others health, safety and security as defined in ourHealth & Safety Policy, to include:

The post-holder will manage their own andothers health & safety and infection control as defined in the SEL GPGroup Health & Safety Policy, the SEL GP Group Health & SafetyManual, and the SEL GP Group Infection Control Policy and publishedprocedures.

Comply with SEL GP Group health and safetypolicies by following agreed safe working procedures

Awareness and compliance with nationalstandards of infection control, hygiene, regulatory / contractual /professional requirements, and good practice guidelines.

Correct personal use of Personal ProtectiveEquipment (PPE) and ensuring correct use of PPE by others, advising onappropriate circumstances for use by clinicians, staff and patients.

Keep updated on developments in care planningand the management of frailty and long-term conditions and consider how toincorporate these into practice.

Work with other advanced clinicalpractitioners to ensure the provision of an appropriate and relevant TARGETtraining programme for the team and support staff to develop their knowledgeand skills.

To lead by example and provide on the jobtraining and development for less experienced members of staff.

To be responsible for maintaining owncompetence to practice through CPD activity and maintaining a portfolio thatreflects personal development.

To participate in annual appraisals as anappraiser and appraise.

To represent the team at a strategic level atthe PCN leadership meeting (upon request)

To be an integral part of the Community PCNleadership team

Governance

Produce complete and accurate records ofpatient consultation, in line with best practice, confidentiality, policiesand procedures

Deliver care according to NHS guidance, NICEguidelines and evidence-based care

Take part in the maintenance of qualitygovernance systems and processes across the PCN and its activities.

Utilise the audit cycle as a means ofevaluating the quality of the work of self and the team, implementingimprovements where required.

Work with other clinical teams on improvingthe quality of healthcare in response to local and national policies andinitiatives as appropriate

Evaluate patients response to health careprovision and the effectiveness of care

Support and participate in shared learningacross the practice and wider organisation

Manage, review and identify learning frompatient complaints, clinical incidents and near-miss events

Awareness of statutory safeguarding,notification processes and local guidance for children/vulnerable patients,applying relevant policies and legislation to protect them

Ensure compliance with policies, proceduresand guidelines for self and others, by taking action or alerting seniormanagement team if the practice appears to contravene policy, or if there areconcerns over any aspect of patient care.

Confidentiality:

Maintain confidentiality of information,always acting within the terms of the Data Protection Act and Caldicottguidance on patient confidentiality.

Maintain an awareness of the Freedom ofInformation Act.

* Demonstrate leadership attributes and motivational skills.

* Substantial specialist experience understanding of how individual and multiple diagnoses affect functional
* performance of older adults
* Evidence of self-directed learning & reflective practice that demonstrate an understanding of current issues
* IT skills, including electronic patient systems, word

* Knowledge and experience of Clinical governance and its implications for therapy services.

Qualifications

* Professional Qualification to degree level or equivalent
* Professional Registration (HCPC)
* Evidence of Specialist Post graduate training related to Frailty or equivalent experience


Other requirements

* Full UK driving licence with access to own vehicle.
