30 hours per week. You will deliver a high qualitycomprehensive mental health service within the Early intervention for Psychosis team. Insight is a team that have been undergoing a lot of development & an exciting opportunity to be part of this growth has come up.

Insight provides well rounded care following the Early intervention for Psychosis standards. We offer those experiencing first episode psychosis a 3 year service &within the 3 years we aspire to offer a well rounded goals based care approach with the involvement of our clients throughout the journey.

Insight offers the following services delivered in house, Set out by the standards of Early intervention. There will be training opportunity for those interested in these areas.

Family intervention

CBT for Psychosis

Employment support

Carers Group

Coffee Group

Acceptance Commitment Therapy

Psychoeducation Group for friends family &carers.

Care coordination working with outside agencies/teams to ensure effective well rounded care is delivered.

Peer support is part of the modal &we are working hard to develop this area.

Insight is a multi disciplinary team that welcomes passionate individuals who are recovery focused &are willing to work in a trauma informed approach to those experiencing first episode Psychosis.


Main duties of the job

The role involves managing a defined caseload of 15 with a focus on providing expert assessment & treatment for people with complex mental health difficulties, primary diagnosis first episode psychosis & also working with their carers who support this client group. This will include recovery co-ordination (design & co-ordination of treatment packages).

You will contribute to the clinical leadership of the team, providing supervision for other members & participating in team/service development & evaluation.

Communication is key & working relationships, being open to feedback & giving feedback in a productive & with an open minded approach.

You will need to travel to engage with people in their own residence or hospital settings whilst planning discharge. They may need to sit for prolonged periods to compile complex reports.

Job responsibilities

JOB PURPOSE

The post holder will deliver high qualitycomprehensive mental health services. The role involves managing a definedcaseload with a focus on providing expert assessment and treatment for peoplewith complex mental health difficulties and their carers. This will includerecovery co-ordination (design and co-ordination of treatment packages).

The post holder will contribute to the clinicalleadership of the team, providing supervision for other members andparticipating in team and service development, and evaluation.

Duties and responsibilities

Communication and Working RelationshipSkills

Contribute to the teams engagement with a widerange of stakeholders including staff, service users, their carers and otherstatutory and voluntary sector agencies.

Impart highly complex and sensitive information to:

Patientsand families/carers re assessment, care planning, treatment and review

Multi-disciplinaryteams to ensure the provision of consistent well-coordinated care

Externalagencies also involved in the provision of care e.g.: voluntary andprivate sector, police, social services, housing, employment etc.

Serviceuser/carer groups

Liaisewith a wide range of other professionals and agencies.

Contribute to the safe and effective day to dayoperations of the service including the handling of clinical enquires.

Provide and receive information some of which maybe contentious, sensitive and highly complex to and from individuals and theirfamilies/carers, where there may be barriers to understanding.

Provide high standards of written and verbalcommunication that is clear and in line with professional documentationstandards

Participate in and chair clinical meetings, thiswill include CPA and non-CPA reviews, professional meetings, and clinical riskmeetings.

Demonstrate the ability to communicate in a waythat is empathic and reassuring whilst employing negotiating techniques to gainco-operation from highly complex individuals.

Communicate information in a way that makes itrelevant and understandable for service users and carers, working in line withpractice standards and operational policies.

Facilitate fair access to social care services andpromote social inclusion regardless of age, ethnicity, gender, sexualorientation or disability

Analytical and judgementskills

Undertake complex risk assessments and develop aformulation of risk which will lead to the development of risk management planswhich have taken into consideration a range of possible options.

Undertake and continuously re-evaluate complexbio-psycho-social assessments and develop a working formulation and diagnosiswhich will lead to the development of personalised care packages in line withevidence based and payment by results.

Ensure the continuous re-evaluation of individualsneeds and clinical risk situations and use clinical judgement to formulate themost appropriate clinical plan.

Ability toreflect and critically appraise own performance, professional and ethicalissues

Planning and Organisational Skills

Organise and facilitate complex meetings, thesewill include: CPA and non-CPA professional and risk meetings.

Responsible for managing their own workload toensure the needs to the individuals who use our services are met taking thewider needs of the team into consideration.

Ensure that interventions are planned in anintegrated and co-ordinated manner particularly where dependent on otherexternal agencies.

Plan and organise individuals packages of care andensure that reviews are held within the required time frames, for exampleSection 117, Funding Panels and CPA.

Plan own workload and the workload of juniorcolleagues to ensure that care is provided in the most high quality and costefficient manner, prioritise and make adjustments as appropriate

Responsibility for Patient/Client Care,Treatment and Therapy

Ensure that caseload is well managed and that careand treatment is provided in line with practice standards and recoveryco-ordination (CPA and non-CPA).

Understand social factors that impactupon mental health.

Develop and deliver care packages includingassessing, planning, intervention and evaluation.

Ensure that all interventions have clear outcomesand that these are taken into consideration when sourcing the resources to bestmeet the individuals needs.

Deliver services using a recovery and strengthsapproach where the needs of the individual and their carers are held as centraland their involvement is proactively encouraged.

Deliver care in line with evidence based practice and employengagement and therapeutic skillsthat are appropriate to the clinical situation presented.

Responsibility for triaging individuals and signposting to the mostappropriate services.

Provide specialist advice to referrers/other multi-disciplinary staffin relation to the care of individuals.

Enable communication and engagement of individuals and their carers toensure that they have a voice about the services they receive and how theseare developed. This may include engagement of advocacy services.

Ensure that appropriate action is taken to safeguarding adults andchildren. This may include providing and receiving highly complex andsensitive information in relation to safeguarding adult and children work,co-ordinating and chairing safeguarding meetings and implementing relevantcare plans.

Ensure that assessment and treatment plans are holistic, outcomefocused and take into account the physical health and social needs of theindividual, including input of significant others.

Ensure the monitoring and review of mental health medication takinginto consideration interactions with physical health needs.

Ensuring timely and accurate documentation of all clinical activity inline with professional guidance and best practice.

Responsibility for Policy and ServiceDevelopment Implementation

Participate in programmes of audit and researchwithin the team as agreed by the Directorate and implement the findings.

Contribute to clinical governance arrangements inorder to ensure the quality of the service provided; this will include ensuringperformance targets are met to provide high quality care.

Involvement in the development of local systems andprocess that ensure the implementation of Trust policies.

Ensure participation in the teams compliance withCQC standards through the participation in provider compliance assessmenttools.

Responsibility for Finance, Equipment and Other Resources

Ensure care is provided in the most cost-effectiveand high quality manner and identify unmet needs.

Order/monitor and maintain any stockrequirements in liaison with the Team Manager.

Undertakemanagement of petty cash transactions as per Livewell Policy.

Responsibilityfor Human Resources, e.g. Supervision, Training, HR Advice and Management

Provision of day to day and clinical supervision asallocated by line manager (this will include participation in the appraisalprocess)

Demonstrate and role model excellentclinical leadership skills.

Assist with the induction of new staff, students and stafftraining as appropriate.

Ensure that core training is maintained and thatthe training and development opportunities in line with the teams WorkforcePlan are participated in to meet professional requirements.

When tasks are delegated, ensure that the personyou have delegated these tasks to is appropriately skilled to do so.

There is a requirement to support learners on aregular basis which includes the achievement and updating of professionspecific training to the required standard to fulfil this role.

Responsibilityfor Information Resources and Administrative Duties

Ensure information resources are used to maintainknowledge and promote a continuous improvement culture.

Ensure information is shared with key stakeholdersand other agencies in a secure manner.

Responsible for maintaining accurateand comprehensive patient treatment records.

Carry outadministrative duties in relation to service provision.

Experience

* Established and relevant mental health experience.
* Experience of managing an extensive and complex caseload of clients with a range of complex mental health problems.
* Working within a community mental health setting in either the statutory or voluntary sector.
* Clinical supervision for qualified and/or support staff
* Experience of working in a multi-disciplinary and multi-agency environment.
* Understanding of current issues within Acute mental health services, clinical governance issues, practice education and implications for nursing.
* Application of health and safety and risk management policies
* Awareness of the implications of long-term institutional care and why early intervention is important.
* Understanding of team dynamics
* Application of HR management policies
