An exciting opportunity has arisen to join the Virtual Ward Team. Virtual wards are a national initiative and the role of the Virtual Ward Clinical Coordinator is pivotal to its everyday function. We are looking for enthusiastic, innovative and engaging individuals who have a passion for the delivery of high quality care within the community. As a Clinical Coordinator you will have delegated responsibility for the clinical co-ordination of patients that are identified as appropriate for admission on to the Virtual Ward service, This will involve assessment of care needs, the development, implementation and evaluation of programmes of care on a shift basis, delivering quality care to patients, who often have complex needs in their own home, as alternative to acute bedded care. This role will be based within Scunthorpe General Hospital.

Main duties of the job

The role will participate in innovation and evidence-based practice through professional leadership and supporting the Virtual Ward clinical team in close liaison with the Acute Consultant for Virtual Ward Advanced Clinical Practitioner, Nursing Associates and the Matron. You are responsible for identification of suitable patients ensuring regular monitoring, reviewing and triage in-line with the patient care plan.

Occupational experience and abilities

Essential criteria

1. •2 years’ post registration experience
2. •Experience of working autonomously
3. •Evidence of working clinically at a senior level
4. •Community or primary care experience
5. •Multi-disciplinary working

Desirable criteria

6. •Team Leadership and Caseload Management
7. •Participation in research projects
8. •Experience in virtual ward services

Knowledge& Skills

Essential criteria

9. •Experience of triaging of patients with a range of clinical conditions and complex needs.
10. •Time management - ability to prioritise and plan ahead
11. •Team working skills with the ability to coordinate others
12. •Demonstrable IT skills

Desirable criteria

13. •Able to demonstrate management competencies

Education and Qualifications

Essential criteria

14. •Professional registration (EG registered nurse, paramedic, AHP)
15. •Evidence of CPD at diploma level in related subject or equivalent experience
