Overview
Leeds City Medical Practice are seeking a dedicated, compassionate and skilled nurse to provide patient-centred quality care as part of a dynamic and supportive team.
The ideal candidate will have a qualification with experience in cervical cytology and immunisations. It would be desirable for experience in Long Term Conditions but the Practice will provide external and internal training as needed. Dependent on experience the position offered will either be for a Senior Practice Nurse or a Practice Nurse.
Interviews will be held on the afternoon of Thursday 20th November 2025.

Management Duties (Senior)
* Working with the Nurse Manager to develop and implement new initiatives, national guidance, policies, protocols
* When the Nurse Manager/Lead Nurse is unavailable to undertake the day to day running of the Nursing Team and any Clinical Issues
* Support and help develop junior members of the Nursing Team
* Lead Nurse for a Long-Term Condition: education for staff, developing new initiatives to improve the service for the practice and patients, auditing the service

Leeds City Medical Practice is a forward thinking, supportive friendly practice that requires an enthusiastic practice nurse who would like to join an experienced Nursing Team delivering evidence-based practice both independently and with the Clinical Team.
We are a large busy Training Practice with ten GP Partners across two sites in Beeston delivering care and services to a population of 19,500 patients. We support education and development of the Nursing Team and have an established Performance and Progression incremental pathway. This develops all the nursing team both clinically and in management skills within a safe, supportive learning structure providing appropriate pay development dependent on nurses skills.
The role requires resilience and the ability to be flexible and adaptable, confident to work independently undertaking autonomous decisions made in practice.

Job responsibilities
JOB PURPOSE
The Practice Nurse will work in accordance with the NMC Code of Conduct with support from the Senior Practice Nursing Team and GPs to deliver practice nursing services both independently and in participation with the Clinical Team. The nurse will deliver high quality patient care by assessment, treatment, screening, health education services and advice to patients. The nurse will work under local and practice policies to ensure patients health needs are met and provide support to healthcare assistants and student nurses as required.
CLINICAL
* All clinical tasks and duties will be performed after completion of accredited courses and deemed competent.
* Assess, plan, implement, and evaluate individual treatment plans for patients with known long-term conditions (e.g., Diabetes, Cardiovascular Disease, Respiratory Conditions)
* Work with other health care professionals to monitor, manage and treat long-term conditions
* Review medication for therapeutic effectiveness and adherence
* Prioritise health problems and intervene in complex, urgent or emergency situations, including initiation of emergency care; competent in anaphylaxis and resuscitation with regular updates
* Support health promotion, health self-management and programmes to promote health and well-being
* Support contraception and cervical cytology needs; provide full contraceptive service following CASH course
* Vaccination and immunisation programmes for adults and children; obtain informed consent and manage vaccine storage and disposal
* Wound care, opportunistic screening, and smoking cessation advice
* Infection control measures and appropriate referrals for sexual health and family planning
* Home visits as required
* Prescribing competencies for Long Term Conditions (Senior) and implant contraceptive procedures (Senior)
* Nurse Prescriber (Senior) and related governance
COMMUNICATION
* Demonstrate sensitive communication to ensure informed consent
* Respect and dignity for patients and relatives; effective communication with patients, carers and MDT
* Record information accurately in patient records and adhere to information governance
DELIVERING A QUALITY SERVICE
* Maintain professional standards and complete patient records
* Prioritise and manage workload; deliver evidence-based care
* Participate in audits, learning across the Practice and discuss significant events
* Understand safeguarding and respond appropriately
TEAM WORK
* Collaborate effectively within the team and participate in local projects
MANAGING INFORMATION
* Use read codes appropriately and follow information governance
CONFIDENTIALITY
* Maintain patient confidentiality and comply with data protection policies

GENERAL
