Community Care Practitioner - Scotland | Sponsorship Available
About the Role
Divine Health & Social Care is recruiting compassionate, reliable, and professional Community Care Practitioners (CCPs) to deliver person‑centred, outcome‑focused support to clients in their own homes. You will help individuals live independently, safely, and with dignity while providing high‑quality care.
As a CCP, you will provide personal care, medication support, and assistance with daily living. You’ll act as a key worker, building meaningful relationships, monitoring wellbeing, and ensuring timely reporting of concerns.
Key Responsibilities
* Deliver care in line with individual Care & Support Plans
* Support clients with personal care, medication, and daily living tasks
* Act as a key worker to build relationships and support positive outcomes
* Monitor changes in clients’ needs, risks, or wellbeing
* Report and elevate concerns to Senior Care Coordinators
* Maintain accurate and timely care records
Candidate Requirements
* Caring, professional, and reliable approach
* Ability to recognise and elevate concerns promptly
* Full UK driving licence and access to a vehicle (essential due to rural locations)
* Right to work in the UK (minimum 3 months remaining)
* Minimum 1 year of home care experience required
Sponsorship
* UKVI‑compliant Skilled Worker visa sponsorship may be available for candidates already residing in the UK
* Applicants must have at least 4 months remaining on their current visa and meet Home Office eligibility requirements
* Divine Health & Social Care does not charge fees for sponsorship
