Main duties of the job
We are looking for an enthusiastic Rehabilitation Assistants to work as part of the Sutton Health and Care at Home service. You will be part of the MDT team that helps to prevent admission from community, accelerate discharge from emergency department and acute medical wards and provide supported discharge through bed and home based intermediate care. You will be working with Nurses, Occupational therapists, Physiotherapists, and Band 4 Rehabilitation assistants in one of the arms of the SHC at Home service. We are innovative and people focused with a 'one team' ethos, which means better quality and more joined up care for our patients and families. The team strives to constantly improve service delivery and contributions from all staff are highly valued. We will provide professional development opportunities and support you to provide great care to our patients.
Driving License is required for this role.

Education/Qualification
Essential criteria
1. Written, verbal and numeric skills to GCSE or similar level
2. NVQ Level 2/3 health related study or equivalent experience
Desirable criteria
3. Cavendish Care Certificate
Experience
Essential criteria
4. Experience of a caring role
5. Experience of working as a member of a team within health or social care
Desirable criteria
6. Demonstrable understanding of the roles of members of the multi-disciplinary team
7. Worked as a therapy or healthcare assistant
Skills/Knowledge/abilities
Essential criteria
8. Demonstrate the understanding of the role of rehabilitation assistant
9. An awareness of health and safety issues
10. Some understanding of issues relating to working in people’s homes
11. Demonstrate initiative, organisational and time management skills, ability and willingness to learn new skills
12. Good written, spoken and non-verbal communication skills
13. Driving license
