Key Responsibilities: Facilitate multi-disciplinary assessments and reviews in line with standard operating procedures. Conduct continuing care assessments for children and young people across primary, secondary, social care, and educational settings. Maintain effective relationships with internal and external stakeholders including social care, education services, and families. Participate in transition meetings (16–18 and 18–25 years) to review care plans and support smooth transitions between services. Contribute to joint commissioning for SEND, focusing on outcomes that matter most to children, young people, and their families. Collaborate with professionals to assess complex needs and develop holistic care plans.
